Payment and InsurancePaymentWe accept most insurance plans and will file your paperwork for your primary insurance carrier. We accept Visa, Mastercard, Discover and American Express It is our office policy to collect co-pays and balances due at the check-in window. Please be ready to pay at the check-in window when you arrive at your appointment. Any self pay fees and/or deductibles will be collected at check-out to ensure proper charges are billed to the patient. InsuranceBelow you will find a list of insurance companies that we accept. If you do not see your company listed, please contact us to see they have been added. Also, placement on the list below does not guarantee that we are in network or that your insurance company will pay for services rendered. Please check with your insurance company to verify that they will pay your medical bills at our office. Any remaining balance will be the patient's responsibility. Reporting the exact services performed and the exact reason for performing the services. We cannot adjust this information just so the insurance company can pay the claim. Our practice is committed to obeying these laws, and we will submit all claims to all insurance companies in this manner We will file to all insurance companies, but we do not have a contract with all insurance companies. Therefore, we are not required to accept what the insurance companies decide to pay out of network. If we have a contract with your insurance company we are required to collect all co-pays and deductibles at the time of service. If you have not met your deductible, you are required to pay for this at the time of service. We will verify that you are covered by insurance before your visits. We are also able to determine what your co-pay and deductible is. Self-PayWe accept self-pay patients, patients who do not have health insurance coverage. We offer to our self-pay patients a 20% discount on all services rendered in our office, if paid in full on the date of service. If the full amount cannot be paid at the time of service, the patient will be billed for the full amount without a discount. If the full amount cannot be paid, this must be discussed with management prior to your appointment. Medicaid & MedicareOur staff at Center for Adult and Family Medicine complies with all billing guidelines established by the governmental authorities and private payers. Our billing personnel reviews all bulletins and newsletters provided to Center for Adult and Family Medicine from Medicaid and Medicare services. Overdue AccountsWe hold all unpaid accounts for 60 days. If your account is not paid in full within this time we will send your account to CBA (a collection agency). If you are in financial hardship and unable to pay the balance in full when you receive your statement please come in to the office and speak with management. We have payment plans for those who qualify. |
